It is important to stay on top of the transcript request deadlines. Find helpful information here.
- College Transcript Request Form
- Please note that Early Action/Decision transcript requests are due in the School Counseling office two full weeks before the college due date. All other requests are due November 15.
- Scholarship Transcript Request Form
- Please note that Scholarship Transcript Request Forms must be submitted to the School Counseling office 2 weeks prior to due date.

You can pay for your transcript by check or by using the PayPal link below. Each transcript request is $4.00. Please print and mail your completed form.
