State of CT Dept. of Education - Health Assessment Record Form
Reporting an Absence
In an effort to minimize the spread of infectious diseases, East Catholic is adopting the following protocol for reporting absences and tracking those related to the flu virus.
Parents must call Student Life 649-5336 ext. 233 and report any absence before 8:30 a.m. Please give your name and your relation to the student. Give the student’s name and homeroom and the reason for the absence. If your child is ill, please report the nature of the illness (flu, fever, sore throat, gastro-intestinal, etc.) If a student has been diagnosed with the flu virus, he/she should follow the instructions of a doctor and not return to school until he/she is without symptoms and not taking medication for 24 hours.
Any child suffering with a fever should remain at home until he/she is without a fever and not taking fever reducing medication for 24 hours. Any child who is ill with symptoms other than a fever should remain home until he/she is without symptoms and not taking medication for 24 hours.
If parents/guardians have any health-related questions, please contact Mrs. DeCarlo, our school nurse, at extension 207.
We thank you for your cooperation in this matter. We pray for a healthy school year for all students and their families, faculty and staff.
